Ismail has published numerous papers on special functions, orthogonal polynomials, approximation theory, combinatorics, asymptotics, and related topics. His well-known book Classical and Quantum Orthogonal Polynomials in One Variable was published by Cambridge University Press in 2005 and reprinted with corrections in paperback in Ismail (2009).

Books for which he has served as editor or coeditor include, Mathematical Analysis, Wavelets, and Signal Processing (with M. Z. Nashed, A. I. Zayed and A. F. Ghaleb), Contemporary Mathematics, v. 190, American Mathematical Society, 1995; Special Functions, q-Series and Related Topics (with D. R. Masson and M. Rahman), Fields Institute Communications, v. 14, American Mathematical Society, 1997; q-Series from a Contemporary Perspective (with D. W. Stanton), Contemporary Mathematics, v. 254, American Mathematical Society, 2000; Special Functions—Proceedings of the International Workshop, Hong Kong, June 21–25, 1999, World Scientific, 2000; Special Functions 2000: Current Perspective and Future Directions (with J. Bustov and S. Suslov), Kluwer Academic Publishers, 2001; Symbolic Computation, Number Theory, Special Functions, Physics and Combinatorics (with F. G. Garvan), Kluwer Academic Publishers, 2001; and Theory and Applications of Special Functions: A volume dedicated to Mizan Rahman (with E. Koelink), Developments in Mathematics, v. 13, Springer, 2005.

Ismail serves on several editorial boards including the Cambridge University Press book series Encyclopedia of Mathematics and its Applications, and on the editorial boards of 9 journals including Proceedings of the American Mathematical Society (Integrable Systems and Special Functions Editor); Constructive Approximation; Journal of Approximation Theory; and Integral Transforms and Special Functions.

Ismail was elected a fellow of the American Mathematical Society in 2014.

In November 2015, Ismail was named a Senior Associate Editor of the DLMF.
